The Eliot Center is seeking a strengths based/trauma informed Outpatient Education and Employment Care Coordinator to provide evidence based services for individuals aged 12 – 35 who are in their first three years of experiencing a primary episode and diagnosis of psychosis.

Our multi-disciplinary team of psychiatrists, psychologists, social workers, mental health counselors and other professionals administer a full range of clinical and support services, resulting in a comprehensive collaborative approach to treatment. The clinician in this role would be required to participate in the Early Episode Psychosis program and team based treatment with a multidisciplinary focus.

Responsibilities:

Supervise medical specialist and/or staff assigned in providing medical/psychiatric supports, intervention, and services

Provide side by side interventions to individuals and their families that focus on education, employment and other case management needs, problem solving, skills training, modeling behavior, optimism and encouragement

Participate in daily Touch Points and weekly Team Meetings

Assist the team and individuals in providing on-going assessment of symptoms and responses to staff interventions

Participate in the development of treatment plans for each individual in which you are a member of their treatment team. Ensure that treatment goals are culturally competent, strength based, realistic, attainable and promote the healing journey from a new diagnosis of psychosis and the many case management needs that might be indicated.

Ensure that each individual is actively involved in their treatment planning process as well as family members when relevant and that their preferences are embraced

Meet program clients and their families as needed in the community to provide care management with education, employment and other socio-economic needs

Qualifications:

Bachelor’s degree in psychology, social work or related field and two years’ experience in case management

Must have a strong commitment to serious mental illness and recovery-oriented services

Ability to communicate clearly and effectively with individuals aged 13 -35 who are experiencing early episode psychosis and comorbidity with other mental illness, such as substance use

Ability to provide flexible services that are community based and focused on team work. This will require a valid driver’s’ license and reliable transportation to engage and support the program population in the community with education, employment and case management supports as needed.
